Athletics Forms & Information
To be eligible to participate in TPS Athletics all student-athletes must create an Athletic Clearance account for the 2025-2026 school year. All paperwork and signatures must be completed and submitted online. Students and parents will receive an email when the student is cleared and eligible to participate. If you have an athletic clearance account from a previous school year, please use the same account. If you are new to Trinity Prep, please create an account.
Athletic Clearance Checklist:
- Create an Athletic Clearance Account or log into your existing account.
- Upload the FHSAA Physical Form.
- Upload proof of medical insurance.
- View NFHS Mandatory Courses – View the three courses at home and then a parent and student will need to digitally sign the FHSAA Consent Form (EL3) in the athletic clearance portal to confirm you have completed each course (courses and instructions below).
- Upload ImPACT Baseline Test – take the baseline test at home, upload to the Athletic Clearance portal (instructions below).
- Review the Student-Athlete and Parent/Guardian Athletic Participation Agreement in the athletic clearance portal. Parent and student will need to digitally sign the agreement in the athletic clearance portal.
- GA4 Affidavit of Transfer Compliance Form – Required for NEW students entering grades 7th, 8th and 10th, 11th, and 12th grade.
Optional Electrocardiogram (ECG) Information – An ECG is not required for athletic clearance. TPS has partnered with Who We Play For to provide every student the opportunity to get their heart checked with an electrocardiogram (ECG).

All Student-Athletes and Coaches are required by FHSAA to complete the three online courses. You will not be able to tryout or practice until courses are complete and the consent form is digitally signed in the athletic clearance portal. You will need to upload your completion certificates to the athletic clearance portal.

The ImPACT test is a computer – based neurocognitive test battery that provides healthcare professionals with objective measure of neurocognitive functioning as an assessment aid and in the management of concussion in individuals ages 12-59.
The ImPACT Baseline test should be completed at home over the summer. Student-athletes will complete a new baseline test each school year.
Please make sure of the following before continuing your test:
- Test environment must be quiet and distraction-free, no phones or other devices.
- Sit comfortably at a table or desk with a flat, hard surface.
- All other programs and browser tabs should be closed.
- An external mouse or a trackpad must be used throughout the test. Touch screens are not acceptable.
- You must complete the test in a single attempt in less than 45 minutes.
Get Started:
- Launch the ImPACT Testing Website, Enter Code: FAY9
- Select Validate
- Select Launch Baseline Test
- Upload your completion page to the Athletic Clearance portal.
